29 CFR 1926.451(g)(4)(iv): Midrails were not installed at a height approximately midway between the top edge of the guardrail system and the platform surface: Worksite located at 6000 W Memorial Road. Oklahoma City, OK 73142. On or about August 03, 2016 and at times prior thereto, employees were exposed to a fall of eighteen feet while working on a fabricated frame scaffold that was not provided with mid-rails.
