29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: On or about May 18, 2016, employees performed maintenance activities on the Unit 12 South Gas Fan and the employer did not ensure all of the employees applied a personal lockout device to the electrical energy source before working on the equipment.
