29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c): Zee Construction Co., Inc. 4930 Woodville Rd, Northwood, Ohio: On or about May 14, 2016 the employer failed to provide protection against cave-in hazards from the sidewalls of the excavation. An employee was struck by soil during a cave-in incident.
