29 CFR 1915.112(b)(3):Where U-bolt wire rope clips are used to form eyes, Table G-6 in 1915.118 was not used to determine the number and spacing of clips: Employees working on the Noble Jim Thompson vessel, set for demolition, at the Port of Brownsville used a 1 1/4" thick wire rope to create an eye to connect to the top of a derrick, and the eye did not have the correct number of U-bolts clips and or spacing.
