29 CFR 1926.600(a)(6)(iv): A person was not designated to observe clearance of the equipment and give timely warning for all operations where it is difficult for the operator to maintain the desired clearance by visual means: On or about October 21, 2015, and at times prior thereto, the employer did not designate a person to observe the distance between the bed of a dump truck and the power lines, exposing employees to electrical shock and electrocution hazards.
