29 CFR 1910.119(l)(3): Employees involved in operating a process and maintenance and contract employees whose job tasks will be affected by a change in the process shall be informed of, and trained in, the change prior to start-up of the process or affected part of the process. a) At the facility in the chlorination department, process operators and maintenance technicians responsible for installing a 3" X 18" stainless steel flex hose on the 20 reactor recirculation line had not been informed or trained on the differences of the two types of flex hoses that were made available to them for replacement use. The company provided a 3" X 18" stainless steel flex hose with a static dissipating PTFE liner and a 3" X 18" stainless steel flex hose with a PTFE liner (non-static dissipating).
