29 CFR 1910.269(m)(3)(iii): Automatically and remotely controlled switches that could cause the opened disconnecting means to close were not rendered inoperable. (a) Morgan County Rural Electric Association at Washington County Roads 20 & E, Woodrow, CO 80757: On or before July 10, 2015, automatically controlled switches that could cause the opened disconnecting means to close were not rendered inoperable, unless its design does not so permit. The circuit breakers for Feeder 1 were located inside the substation adjacent to the work area and the automatic re-closers were not rendered inoperable. On July 10, 2015 an employee was electrocuted when he came into contact with 7.2 kV, phase to ground. This condition exposed employees to electric shock and/or an electrocution hazard.
