29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in the sequence as required by 29 C.F.R. 1910.147(d)(1)-(6): In the production areas of the Treif #1 and #2 machines, the machine operators were exposed to an amputation hazard while unjamming the swirl roll product by opening the plexiglass cover on an inadequate machine safeguarding system.
