Warehouse, T&R Wholesale Foods, 745 Springfield Street, Feeding Hills, Massachusetts; The employer used three separate, 100 foot extension cords, run from outlets by the dock doors, through the overhead, and down to work areas to provide power for equipment in the warehouse.
