29 CFR 1910.146(d)(4)(iii): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not ensure that employees used the communications equipment properly: a) Thomas Concrete of Georgia Plant # 500, Wash Grade Sand Silo # 2; 4650 Canton Road, Marietta, GA: The employer did not provide proper communication equipment and/or ensure that it was available for effective communication between the attendant and authorized entrants inside the silo, to monitor status and to alert entrants of the need to evacuate the space when hazard exposure was known or detected.
