29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels : a) Grinding Building - The grinder operator who was exposed to continuous noise at 301% of the permissible daily exposure limit (8-hour time weighted average sound level of 90 dBA) or an equivalent sound level of approximately 97.9 dBA during the 395 minute monitoring period had not had an audiogram since February 2008. Violation discovered on or about March 5, 2015.
