29 CFR 1910.135(a)(1): Each affected employee did not wear a protective helmet when working in areas where there was a potential head injury from falling objects: (a)Karl W. Schmidt & Associates, Inc. at 3900 E 68th Ave, Commerce City, CO 80022: On and before February 13th, 2015 the employer did not ensure that employees wore head protection when they worked near and under elevated forks of industrial trucks. Forklifts were used to lift conveyor belts 6 to 8 feet above the shop floor while employees drilled holes and installed hardware onto the other end of the belt which hung down from the forks. This condition exposed employees to being crushed or struck-by belts that weighed up to 3,000 pounds.
