29 CFR 1910.217(b)(13): Control system(s) of mechanical power press(es) were not constructed so that a failure within the system would prevent the normal stopping action from being applied to the press when required, but would prevent initiation of a successive stroke until the failure was corrected; a.) Shiloh Industries, Inc. dba Wellington Stamping located at 350 Maple St. Wellington, OH 44090; On or about February 5, 2015, the employer did not assure employees were protected while performing hand in die operations on presses, including, but not limited to the 01C and 01D Danly OBI Mechanical Power Presses. Employees are required to operate presses in INCH mode for production and while adjusting the top stop fault and are exposed to amputation hazards in that they are not control reliable.
