29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: This violation occurred on January 21, 2015, and at times prior thereto, in the maintenance area, employees were operating an Amrox bench grinder model # 7550A, s/n not legible 8 in diameter wheel with no tongue guards installed on both sides exposing employees to struck by hazards.
