29 CFR 1910.27(b)(1)(v): The rungs of all individual-rung ladders were not designed to prevent the foot from sliding off the ends: (a)Cyclone Rig #17, 05-045-22491-00, Parachute, CO, 81635: On and before October 21st, 2014, the employer did not ensure that the rungs of all individual-rung ladders were designed to prevent the employees foot from sliding off the ends. The ladder consisted of 1-1/2 inch angle iron used as ladder rungs for accessing to the top of each "A" leg. The ladder was not designed and installed to prevent the user from slipping off the ends. The condition exposed employees to cuts, lacerations, and bruises while climbing the two ladders. Abatement Note: Among others, one suggested design may be found in Figure D-1 in 29 CFR 1910.27
