Text For Citation: 01 Item/Group: 003B Hazard:
29 CFR 1926.454(b)(2): The employer shall have each employee who is involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question. The training shall include the correct procedures for erecting, disassembling, moving, operating, repairing, inspecting, and maintaining the type of scaffold in question: Dusit Thani Hotel Jobsite, Sixth Floor, Banquet Room 1 and 2: Pacific Rim Land Development permitted a four tier fabricated frame orange mobile scaffold to be moved by employees that had not received training specifically on the hazards associated with moving a mobile scaffold. On October 18, 2014, the mobile scaffold toppled over after an employee attempted to push the scaffold over a three quarters of an inch diameter electrical cord fatally injuring a plumber. The scaffold was hazardous in that the scaffold frame legs were not designed with holes to accept mobile scaffold wheel stem pins to prevent the wheels from coming out of the frame legs; and the height to base ratio of the mobile scaffold was greater than 4 to 1. When the orange scaffold toppled it was measured to be 16 feet, 7.5 inches tall and 4 feet wide.
