29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: P&W Painting Contractors, Inc. - Toledo, Ohio: On or about October 1, 2014, the employer failed to obtain approval from the forklift manufacturer before modifying the mast of the JCB 930 forklift, affecting the capacity and safe operation of the lift.
