29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: On or about August 27,2014 in the Brown and Shape area, employee using the Baldor pedestal grinders was exposed to a struck-by hazard when the work rest opening was greater than 1/8 of an inch.
