29 CFR 1910.119(e)(3)(i): The Process Hazard Analysis did not address the hazards of the process (a) 1 Devco Drive, Manchester Pennsylvania - The employer did not ensure that the process hazard analysis included all appropriate elements of the system. The process hazard analysis did not included such items as but not limited to analysis of high or low pressures at each vessel, on or about August 20, 2014. (b) 1 Devco Drive, Manchester Pennsylvania - The employer did not ensure that the process hazard analysis included all appropriate elements of the system. The process hazard analysis did not include such items as but not limited to analysis of the failure of a nitrogen system, on or about August 20, 2014. (c) 1 Devco Drive, Manchester Pennsylvania - The employer did not ensure that the process hazard analysis included all appropriate elements of the system. The process hazard analysis did not include such as but not limited to analysis of the failure of the fluorine regulator, on or about August, 20, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
