29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees: without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph. a) Jobsite: West scaffold - On or about 5/20/14, an employee conducting masonry operations used a portable ladder on the 4th working platform of the supported tubular welded frame scaffold, approximately 26 feet above the lower level, without securing the scaffold against the lateral thrust exerted by the ladder, securing the platform units to the scaffold, or providing other means to stabilize the ladder against unequal platform deflection.
