29 CFR 1910.147(d)(3): Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). (a) During a product line change of the 72 inch Braner Slitting Line, affected employees removed scrap from the back of the little shear on the 72 inch Braner Slitting Line without first isolating the energy sources, thereby exposing them to the unexpected start up of the shear.
