29 CFR 1910.215(a)(4): Grinding machinery was not used with work rest(s) to support offhand grinding work: On or about April 25, 2013, and at times prior thereto, in the grinder areas in front of maintenance shop the employer allowed employees to operate abrasive wheel grinding machine which did not have work rests provided. Employees were exposed to flying fragments in the event of an exploding wheel.
