29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: a. On or about February 5, 2013, in the maintenance shop, the grinding machine did not have a work rest on the left side and on the right side the work rest was adjusted to 1/2 inch, exposing employees to a struck by hazard.
