29 CFR 1910.134(d)(1)(i): Selection of appropriate respirators was not based on the respiratory hazard(s) to which the worker was exposed and user factors that affect respirator performance and reliability: a) On or about November 29, 2012, Phoenix Industrial Cleaning, Inc. did not select an appropriate respirator for employee(s) use that was based on the hazards of an oxygen deficient atmosphere and methylene chloride, prior to entry into a 6,000 gallon 16 foot vertical chemical storage tank at Sunnyside Corporation in Wheeling, Illinois. This tank was last used by Sunnyside Corporation to store methylene chloride.
