29 CFR 1915.111(a): All gear and equipment provided by the employer for rigging and materials handling was not inspected before each shift and when necessary, at intervals during its use to ensure that it is safe; defective gear was not removed and repaired or replaced before further use: a) On 04-13-12, employees who were hoisting equipment, Motor Control Center, onto the Kay E. Barker ship had removed the latches from the Kuplex chain sling hooks and were using the chain slings to hoist the equipment. Abatement Certification and Abatement Documentation required
