29 CFR 1910.119(f)(1)(i)(D): The employer's written operating procedures covering the steps for each operating phase did not address emergency shutdown including the conditions under which emergency shutdown is required, and the assignment of shutdown responsibility to qualified operators to ensure that emergency shutdown in a safe and timely manner: a)At the establishment, the employer did not develop or implement operating procedures with clear instructions related to the emergency shutdown control switches located on the outside of the two engine rooms. Employees were exposed to catastrophic release of ammonia which result in asphyxiation, explosion and flash fire leading to death and injury to persons in and near the facility. b)At the establishment, all of the operating procedures for ammonia process equipment did not include any revision to explain how to determine when it is safe to enter the engine room or what personal protective equipment to wear before entering to isolate and purge leaking equipment. Employees were exposed to catastrophic release of ammonia which results in asphyxiation, explosion and flash fire leading to death and injury to persons in and near the facility. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.
