29 CFR 1910.119(e)(3)(i): The process hazard analysis did not address the hazards of the process: Company: The Process Hazard Analysis conducted by the company of the ammonia refrigeration system did not address hazards associated with purging equipment of ammonia prior to opening piping and equipment. Employees routinely purged ammonia from piping and equipment using a hose and barrel of water, sometimes setting off the ammonia sensor in the mechanical room.
