Text For Citation: 01 Item/Group: 003A Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: hazard of being struck by automatic guided vehicles operating in aisles of the facility: a)Worksite; US Postal Service, 1050 Forbell Ave, Brooklyn, N.Y: Collision avoidance equipment on the AGV (automatic guided vehicles) was incomplete. There was a "blind spot" at turns where the electric eye and sensor bumper would not detect a person or object standing at the arc of the turn. They could be struck by the side of the machine as it "sweeps" while turning; on or about 7/18/01. Additionally, the sensors did not prevent collision with the racks on the back of the Tugger machines due to the positioning of the AGV sensor bumper and electric eye; on or about 7/18/01. One feasible means to correct the hazard is to provide additional collision avoidance equipment as required by industry standard ANSI B56.5-1993 par 8.10.2 (g) which requires "a sensing device or combination of devices to prevent contact of the object sensed with the vehicle structure in the direction(s) of travel". NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 AS PER 29 CFR 1903.19.0
