Text For Citation: 01 Item/Group: 003A Hazard:
29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not using electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed: a.) Ellwood City Forge Company; 800 Commercial Avenue Ellwood City, PA 16117, on or about 08/24/2025, and times prior thereto: Employees were working inside the General Electric 4160 volts T2/T3 switchgear in the Millwright Shop Room. Safety-related work practices including, but not limited to, the use of appropriate electrically rated personal protective equipment were not used by employees. Employee was not wearing an arc flash rated jacket and face shield while working inside established working distance of the energized 4160 volt switchgear. b.) Ellwood City Forge Company; 800 Commercial Avenue Ellwood City, PA 16117, on or about 08/24/2025, and times prior thereto: Employees were working inside the General Electric 4160 volts T2/T3 switchgear in the Millwright Shop Room. The switchgear was not de-energized prior to performing work operations. Employee was not wearing an arc flash suit while beside employee working inside the established working distance of the energized 4160 volt switchgear. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.
