29 CFR 1910.146(d)(6): The employer did not provide at least one attendant outside the permit space into which entry was authorized for the duration of entry operations. On or about 25 June 2025, Gilster-Mary Lee Corporation, located at 615 Old St. Marys Road in Perryville, Missouri, did not provide at least one attendant outside of a known permit-required confined space during entry. An employee entered Sugar Coat 1 Oven to perform scheduled cleaning while the remaining crew continued to work on other cleaning tasks.
