29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity): a) Jobsite located at or near 5800 City Ave. Philadelphia, PA- On or about June 24, 2025, employees were exposed to fall hazards of 60 feet in height while installing masonry units from outriggers on a Fraco Act 8 Mast Climbing Work Plaform that lacked guard railing on the ends. b) Jobsite located at or near 5800 City Ave. Philadelphia, PA- On or about June 24, 2025, employees were exposed to fall hazards of 60 feet in height while installing masonry units from outriggers on a fabricated frame scaffold that lacked guard railing on the ends. c) Jobsite located at or near 5800 City Ave. Philadelphia, PA- On or about June 24, 2025, employees were exposed to fall hazards of 60 feet in height from a fabricated frame scaffold that lacked guard railing on the end frames while installing masonry units. IN THE ALTERVINATIVE 29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms.
