29 CFR 1926.1053(b)(15):Ladders shall be inspected by a competent person for visible defects on a periodic basis and after any occurrence that could affect their safe use. a. Located at or near 6050 East Broad Street, Columbus Ohio, 43213: On or about April 30, 2025, the employer did not ensure that the ladder on the jobsite was inspected by a competent person for visible defects prior to being used. The employees were using a ladder to enter/exit a trench that had bent and missing rungs. These conditions exposed the employees to falls of up to 12 feet.
