Text For Citation: 01 Item/Group: 003A Hazard:
29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not capable of supporting at least 5,000 pounds per employee attached. Location: Job site; On or about 04/14/2025, employees were allowed to utilize temporary roof anchors which were not installed per manufacturer instructions to support 5,000lbs per employee in the following instances: a) an anchorage point near the front of the residence had multiple missing fasteners and fasteners which were not fully secured to the anchorage point and the roof surface they were affixed to. b) anchorage point at back of house which affixed to the hipped section of roof and ran along ridge beam had multiple missing fasteners in the leg of the anchorage point which was running along the ridge beam. c) anchorage point installed on section of the garage/shed/ancillary building at back of the residence had only a single fastener affixed to the leg overhanging the edge of the roof. This fastener was also not fully secured and was bent back over the overhung leg at the edge of the roof.
