29 CFR 1910.147(c)(4)(i):Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. a) Located at 150 Fourth Avenue, Mount Sterling, OH 43143: On or about November 13, 2024, the employer did not ensure employees utilized the energy control procedure to perform servicing and maintenance activities, including setting up the extruder. Employees did not perform lockout/tagout and were exposed to pinch point hazards as a result. b) Located at 150 Fourth Avenue, Mount Sterling, OH 43143: On or about November 12, 2024, the employer did not ensure employees utilize the energy control procedure to perform servicing and maintenance activities to include removing vent needles. Employees did not perform lockout/tagout and were exposed to pinch point hazards as a result.
