29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about October 23, 2024, at a worksite located next to 1057 Pearl RD, Brunswick Ohio 44212; during the removal and installation of new signage at a commercial store front, the employer did not ensure all employees using an aerial lift (Van Ladder Bucket Truck) were protected from fall hazards approximately 10.5 feet by a fall protection device.
