29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. On or about October 18, 2024 and at times prior thereto, on the east side of the warehouse, employees perform modifications and maintenance on the preexpander and DWS hopper without affixing lockout devices to all sources of energy.
