29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) On or about October 3, 2024, and at times prior thereto, on the cartoner machine M80, employees were removing jammed papers on the cartoners without applying locks and/or tags to isolate the energy sources. b) On or about October 3, 2024, and at times prior thereto, on the cartoner machine M61, employees were removing jammed papers on the cartoners without applying locks and/or tags to isolate the energy sources. c) On or about October 3, 2024, and at times prior thereto, on the cartoner machine M66, employees were removing jammed papers on the cartoners without applying locks and/or tags to isolate the energy sources.
