29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high: a)Heartland Tank Inc. at 21242 461st Avenue, Volga, SD 57071: On or about and at times prior to September 7, 2024, the employer did not ensure adequate guardrail systems were in place while conducting work on the top catwalk of the Harvestore silo in that no mid-rail was in place. This condition exposed employees to fall hazards greater than six feet.
