29 CFR 1910.147(c)(7)(i)(A): Each authorized employee did not receive training in the recognition or applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: Employees engaged in servicing and maintenance activities at the following equipment in the fabrication department shop/area, and throughout the different buildings of the plant were exposed to burn, electrical, struck-by, caught-in and caught-between hazards in that machine specific energy control procedures training had not been provided to maintenance personnel to recognize the hazards of unexpected energization or start-up of the machines or equipment, or release of stored energy, which could harm employees. Employees perform servicing and maintenance activities on equipment including, but not limited to, Rousselle Press machine (Model 6F; Serial FF-509-64), Rousselle A4 Twisted Die Press machine (Model 3; Serial DA-6904), and Summit 100-Ton Press machine (Model A9) which had multiple energy sources including pneumatic, mechanic, electric, and gravity.
