29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained, or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices: a) On or about 08/06/2024, in the Medical Records Room, when maintenance employees were servicing the AC12 air handler's return fan motor, residual potential energy was not relieved or restrained causing the motor's belt to turn, creating a pinch point. When the employee was checking the belt tension their finger got caught between the belt and the pulley causing a partial finger amputation. ABATEMENT DOCUMENTATION REQUIRED
