29 CFR 1910.184(e)(2)(ii): Makeshift links or fasteners formed from bolts or rods, or other such attachments, shall not be used. a. STAR Turbine, Inc. - Marion, Ohio: On or about August 1, 2024, the employer used a twist-lock carabiner as a lifting attachment to lift packing ring parts in Bay 2. Employees that used the makeshift lifting attachment were exposed to struck-by and crushing injuries in that the attachment was not designed and manufactured as a lifting device. No additional abatement information is needed for this item.
