29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. Location - Establishment's written LOTO program: a) On 05-10-24, LOTO procedures do not instruct authorized persons how to control the gravitational energy that could cause struck-by or crushing injuries to hands or fingers while removing and re-installing punch holders in the upper die of presses.
