29 CFR 1910.146(c)(4): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146: On June 3, 2024, at the job site located at 913 N Odell Ave, Marshall, Missouri 65340, employees were exposed to engulfment hazards. The employer did not develop and implement a written permit space entry program. Employees were working inside of a fully loaded trailer hopper to free hung up (stuck) blood meal with during the offloading process.
