29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level: a) 4701 Pine St, Philadelphia, PA 19143 - On or about April 11, 2024, the employer did not ensure each employee working on a scaffold 10 feet above a lower level were protected from fall hazards. An employee was required to perform masonry facade work from a scaffold platform without guardrails or other means of fall protection, thereby exposing the employee to falls approximately 13.5 feet from the ground.
