29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department, or other group: a) Butterfinger Kitchen - On or about February 5, 2024, the employer did not ensure that the established group lockout/tagout procedure was utilized when multiple employees were performing servicing tasks on equipment, including cleaning and pre-startup sanitation on section 3 of the Butterfinger Line bar former's slitter and gauging roller.
