Text For Citation: 01 Item/Group: 003A Hazard:
29 CFR 1910.178(k)(1): The brakes of highway trucks shall be set and wheel chocks placed under the rear wheels to prevent the trucks from rolling while they are boarded with powered industrial trucks. a) On or about 2/05/2024, employer did not ensure that the brakes of a semi-truck(US DOT# 2608851 , KYU# 354735) were set to prevent the truck from driving away while a Hyster forklift ( SN# A268N06609J) and it's operator attempted to board the back of the truck at bay dock door 2 in outbound. The truck pulled off while the operator was entering the truck as they were loading a pallet exposing the operator to a 4ft fall from the dock. b) On or about 2/06/2024, employer did not ensure that the wheel chocks were placed under the rear wheels of semi-truck docked at bay dock door 2 in outbound during loading operations. A truck was observed running over the chock as it was pulling off.
