29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a)Lloyd Companies dba Engineered Truss Systems, LLC. at 1400 East Benson Road, Sioux Falls, SD 57104: On or about and at times prior to January 17, 2024, the employer did not ensure that employees were protected from rotating parts and pinch points in that guarding was not installed over tooth-on-chain delivery mechanisms on Saw Machine 1 in the Saw Room. This condition exposed employees to amputation hazards. (b)Lloyd Companies dba Engineered Truss Systems, LLC. at 1400 East Benson Road, Sioux Falls, SD 57104: On or about and at times prior to January 17, 2024, the employer did not ensure that employees were protected from rotating parts and pinch points in that guarding was not installed over tooth-on-chain delivery mechanisms on Saw Machine 2 in the Saw Room. This condition exposed employees to amputation hazards. (c)Lloyd Companies dba Engineered Truss Systems, LLC. at 1400 East Benson Road, Sioux Falls, SD 57104: On or about and at times prior to January 17, 2024, the employer did not ensure that employees were protected from pinch points in that guarding was not installed over pneumatically powered retracting arms on Roll/Press Machine 1. This condition exposed employees to amputation hazards.
