29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: (a) 938 Washington St., Allentown, PA 18102 -2nd floor: On or about December 21, 2023, a 4 inch receptacle was not utilized per the manufacturer's labeling and listing requirements in that it was improperly installed, wired, and protected. (b) 938 Washington St., Allentown PA 18102 - 2nd floor: On or about December 21,2023, a flexible cord was improperly connected to a feed source to provide temporary power for portable tools and equipment. The cord was energized and not utilized per the manufacturer's labeling and listing requirements.
