29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: On or about December 16, 2023, and at times prior thereto, employees required to perform lockout on machinery with multiple energy sources were not provided with enough locks and lockout devices and/or adapters needed to successfully isolate each energy source, exposing employees to amputation, caught-in, struck-by, and thermal hazards.
