Text For Citation: 01 Item/Group: 003A Hazard:
29 CFR 1910.178(l)(4)(i): Refresher training, including an evaluation of the effectiveness of that training, shall be conducted as required by paragraph (l)(4)(ii) to ensure that the operator has the knowledge and skills needed to operate the powered industrial truck safely: On or about January 8, 2024 employees engaged in cold storage activities at United States Cold Storage located at 4302 South 30th Street, Omaha, Nebraska were exposed to crushing and struck-by hazards in that the employer did not conduct refresher training and/or an evaluation of the effectiveness of training to ensure the operator had the knowledge and skills needed to operate the powered industrial truck safely. Instances included but were not limited to: i. Operator(s) involved in an accident or near-miss incident and, ii. Evaluation of each powered industrial truck operators' performance at least once every three years.
